Dental Treatment 50/50 Plan-When patients elect treatment, and for the patient portion we will gladly divide the payment to reflect 50% at the initial visit and 50% at the time you reserve the final appointment. Payment may be made by using cash, check, Visa, MasterCard, or Visa.
Insurance Co-Payment-We are more than happy to help you maximize any insurance benefits that may reduce your out-of pocket expense for dental treatment! Please be aware that once your insurance sets their “Usual & Customary” limit, any difference between this amount and our fee will become your responsibility. We cannot guarantee your insurance plan estimate and there may be a remaining balance owed by you. Please remit payment within 30 days from date of statement.
